Similarly to changeset #54543256, please make sure that your changeset messages reflect what actually changed; and phone numbers should use the country code and be formatted in three groups.

I can't make sure of anything. Mistakes happen - to you even I bet! The browser editor pre-fills in with the last choice, and if I forget to change it, then I forget, simple. The program does not allow a retrospective edit of the note - that's perhaps where effort should be directed?. So that's that I'm afraid.

The browser does indeed fill in the last changeset message, but it’s easy enough to take a few seconds to look over the fields above the ‘Upload’ button in iD before pressing the button. You could also, for example, fill in the hashtags and sources fields to provide more information about your changes. We all make mistakes, but we can all also try and find ways of making sure those mistakes don’t happen again. :-)

My point still stands that any mistake or edit on OSM can be re-edited and fixed. Except this one of changeset descriptions.
I, like everyone could do more of this or that. But my balance of incentive and time generally drives me to what I do. no more no less. I generally tidy up rural areas cos it's my thing, and it gets little attention by others. To do more of what you ask would make it not cost effective for my "mind" and I would cease working on OSM. Each change usually affects streams, quarries, farms, isolated dwellings woods etc. and I'm never going to document each one as it's a waste of my time..
