Please have a look at <https://wiki.openstreetmap.org/wiki/Tag:amenity%3Dtoilets#Gender_neutral_toilets>. Because unisex=yes has been used as a shortcut of female=yes + male=yes, this tag doesn't necessarily imply that the toilets are gender neutral.

'unisex' was always defined as something non-segregated. If you see a reason to introduce a new tag with exactly the same meaning (and redefine and dilute the meaning of unisex at the same time), I suggest to write a formal proposal.

Some other people came to other conclusions:

Even the iD editor lets you only choose between either female, male or unisex. And when i tagged a toilet as female=yes + male=yes + unisex=no in the past, there were people changing it to unisex=yes ...
I don't want to dilute the meaning of the unisex tag – it is already diluted. I'm thinking of writing a proposal, but if people delete the gender_segregated tag, it will likely be rejected because of no use ... -

Usually we try to start a project to clean up existing data if we find a problem how a tag has been used in the past.
If you feel the urgent need to add a new tag, feel free to do so. But note that this still requires to check all "unisex" tagged objects, whether they used the actual definition of the tag or the wrong scheme you assume they use.
