The problem with place names in the Kaliningrad Oblast is that they have an old German name, written in the Latin alphabet, and a new Russian name, written in the Cyrillic alphabet. What used to be Königsberg is now Калининград and that's just fine. However, the old_name:de entry in the Properties table does not make the name of Königsberg visible on the map. The German Place Name field appears to be solely used for transliteriation purposes, so what you see there is 'Kaliningrad' only. In order to dig out Königsberg from the Properties table and throw it onto the map, one has to make it a duplicate entry in the German Place Name field. If you log into openstreetmap.de and take a glance at Poland you shall see how magnificiently the German and Polish place names come simultaneously into view as you zero in on them. The same should be true for Калининград / Königsberg / Kaliningrad.

It is problem of render not data.
Old names should be in appropriate old_name:* tags. Current names should be in appropriate name:* tags
Write to openstreetmap.de to ask include support of old_name:de tag on it render -

The same should be valid and in Poland.
Old historical names couldn't be placed as actial names.
